Travel: Travel is included up to a 3-mile radius. Service areas exceeding a 3-mile radius are subject to $10 charged in
15-minute increments. Travel fees are paid separately directly to practitioner.

Claims/MedPay/PIP: All services are cash-pay upfront. Detailed Superbills (for select massage/bodywork services) with CPT/NPI coding are provided upon request for out-of-network health insurance reimbursement, MedPay and Workers' Comp claims, or HSA/FSA. Doctor's prescription required for HSA/FSA and insurance claims. Select services within treatment plans may qualify for partial FSA/HSA reimbursement with appropriate CPT codes.
